Dear Insurance Agent:
This fax is requesting a signed Certificate of Insurance on the below name insured. The certificate should list the carrier's coverage for cargo and auto liability. The Certificate requires the following information:
  1. Auto liability: $1,000,000 Minimum coverage.
  2. Auto liability: $1,000,000 Minimum coverage.
  3. The following listed as additional insured with a
    30 days cancellation notice.
